Form Builder
Build custom intake forms with 10+ field types and conditional logic.

The Form Builder lets you create custom intake forms, waivers, and questionnaires that clients fill out during booking.
Field Types
Over 10 field types are available:
- Short text
- Long text
- Phone
- Dropdown
- Checkbox
- File upload
- Signature
- Date
- Time
Conditional Logic
Show or hide fields based on a client’s previous answers. For example: “Do you have any allergies?” — if Yes, show a text field to describe them.
Customization
- Drag and drop to rearrange fields.
- Mark any field as Required or Optional.
- Collect digital signatures for waivers and consent forms.
Go to Settings > Form Builder to create a new form. Forms can be linked to specific services or applied globally.
